Description:

We are looking for a Medical Social Worker to provide care at our Patient's homes in primarily Solano County, and some Contra Costa Counties. As a Medical Social Worker, you will assist patients and families in coping with problems resulting from severe or long-term illness. You will assess, diagnose, and treat patient's mental and social conditions, counsel individuals and/ or families, and update case records.

Requirements:
  • Assesses psychosocial status of patient and social/ emotional factors related to the patient's illness, need for care, and response to treatment.
  • Assisting patients and families with goal setting, counseling, and planning and providing interventions.
  • Assisting patients and families with appropriate community recourses, their eligibility criteria, and regulations.
  • Participates in the development and periodic renewal of the patient's plan of care.
  • Observes, and reports to the physician the response to the patient's treatment.
  • Maintains verbal and written communication with other personnel involved with the care of the patient.
  • Maintains appropriate and timely patient care records in the EMR, including observation, treatments, patient response to treatment, and change in patient's condition
  • Attends and participates in appropriate patients care conferences, staff meetings, and agency committee meetings.
  • Ability to establish and maintain effective working relationships with patients and families, and staff members.
