We are currently seeking an experienced Registered Nurse to join our dynamic team supporting Internal Medicine located at the Kenmore , Boston practice of Atrius Health.The Registered Nurse supporting Internal Medicine provides practice management support to a clinical team, recognizing the patient as the central focus. Provides telephone advice to patients with minor acute problems. Serves as a patient educator in areas related to self-care. Supports the on-going management of patients with stable chronic diseases. Monitors acutely ill patients during the patient visit. Performs a variety of procedures that within the scope of the professional nursing practice.
Day to day includes phone triage, education and chronic disease management. Strong focus on titration of blood pressure and diabetes medications by protocols with PCP/AC referrals. May assist with dressing, injections or procedures.
Education: Graduate of a state approved school of nursing. BSN preferred. Must maintain Basic Cardiac Life Support (BCLS) certification.
Skills and Experience: Current/Valid license to practice professional nursing in the Commonwealth of Massachusetts. Minimum of 2 years Nursing experience in intermediate medical/surgical care, Urgent Care or Ambulatory specialty. Proficient computer skills required. Excellent phone triage and assessment skills. Experience in coordination of In -Patient hospitalization through post-discharge outreach. Ability to support the organizational care model of providing excellent patient care.
